Confidentiality
Your information is strictly confidential and will be guarded closely by our staff as private. We will not release any information from your file without your written permission. Communication forms are available for patients to advise our staff how they would like to be contacted regarding appointments and test results. Appointments
The practice schedules appointments Monday through Friday. If you are a new patient to our practice, please bring your current insurance card and arrive at least 15 minutes prior to your scheduled appointment to allow time for completion of paperwork.Click here to print our Patient Registration form and click here to print our Medical History form. Please complete this information prior to your first visit and bring along with you on your visit. We appreciate your patience in giving our staff time to accurately add your information to our files. It is important that your doctor has this information since certain medical conditions can require a different approach to your treatment. We make every effort to stay on schedule; however, sometimes delays are unpreventable. We appreciate your understanding any delays during those times. Our staff will make every effort to see that you receive the same personalized attention that we provide to all our patients. We will notify you promptly if your appointment will be delayed. Should you find it necessary to cancel or reschedule your appointment, we ask that you give us at least 24 hours notice so that we can offer this valuable time to another patient. Financial Policies
Our practice accepts Visa, Master Card, and Discover for your convenience. We also accept personal checks. We deliver the finest care at the most reasonable cost to our patients, therefore, payment is due at the time service is rendered unless prior arrangements have been made. In most cases your insurance plan requires you to make a co-payment at your visit. We will ask you for your co-payment upon check-in. Please have your current insurance ID card available at each visit so we can avoid insurance filing errors. We will bill your insurance as a courtesy to you, our patient, but you are ultimately responsible for any balances that may not be paid by your insurance.
Living Will
A living will (or medical directive) is a document that instructs your doctor or care provider about your preferences for life-saving procedures in the event that you have a terminal condition or are incapacitated. For example, you can decide whether you want your doctor to initiate life-sustaining procedures such as CPR, administer artificial nutrition and hydration or keep you as comfortable and free of pain as possible if you have a terminal condition or are in a unconcious state.
